HVMC TRANSPORTATION LLC CLAIM SUBMISSION FORM

The following documents must be uploaded for processing of your claim. Failure to provide required documents may delay your claim or result in denial. If you do not have the required documents please come back to this page and submit your claim when you do.

Do not disposed of any damaged product. It may be necessary for the carrier or insurance investigators to inspect the alleged damage. Disposal of the product before resolution of your claim or before inspection and release for disposal will result in permanent denial of your claim.

Any claimant is legally obligated to take all reasonable steps to mitigate or otherwise reduce or limit the financial impact or damages resulting from any claim. This includes if possible liquidating damaged but usable or salvageable products at reduced pricing and reducing the claim amount by the amount recovered from these efforts. Failure to meet this legal obligation can have an impact on your claim.

49 CFR 371.3 of the Federal Statute requires all brokers to maintain records of the following;

1. Name and Address of the Consignor

2. Name and Address of the originating motor carrier

3. The Bill of Laiding or Freight Bill Number

4. The amount of compensation received by the broker and the name of the payer.

5. Description of any non-broker service performed in connection with each shipment and the amount of compensation received for the service, the name of the payer, and

6. The amount of any freight charges collected by the broker and the date of payment to the carrier.

Brokers are required to keep these records for a period of 3 years, and

Each party to a brokered transaction has the right to review the record of transaction required to be kept by these rules.
